Everyday Nutrition Policies:
In order to meet your needs and provide you the best possible care in a timely and efficient manner, we ask you to honor the following guidelines:
1) Payment should be received before or at the time of appointment in the form of cash, check or credit.
2) All outstanding balances will be billed to you. Late fees will be incurred after 30 days. Your account will be sent to collection if not received in 90 days and will include any collection fees and late fees you have incurred.
3) A 24 hour notice must be given for all cancellations of appointments. If 24 hours is not given, you understand that you will be billed a $25.00 fee. Everyday Nutrition agrees to give you 24 hour notice for cancellations, as well.
4) Everything that you discuss with your nutrition coach will remain completely confidential. If you choose to discuss your session your coach with others, this is completely up to you but your coach will not share our work together with others. However, coaches are not bound by client/patient privilege laws and maybe required to report legal information to authorities.
